Closed a clinic I was using for 6 months in Oregon. Only found out the clinic was closing due to calling in about a document I requested from the doctor that wasn't done yet. Was told the document (letter about my treatment) wasn't done and might not be done at all because clinic was closing. This was the first I was told about it. The staff also threatened to withhold my final prescription because of an outstanding balance of 25.00 dollars for my med records. I had tried a week previously to call and pay the balance and left a vm asking for a call back which was never followed through on. The doctor shouldn't have agreed to write a letter that was needed for legal reasons if he never intended on doing so. It's unprofessional and has stalled my attorney from doing his work because he was waiting for something the doctor said would be done weeks ago.
