I was due for my second annual cleaning provided through Compbenefits, my dental provider. With about a week's notice I was informed by mail that Dr. Alvarez was no longer accepting my insurance. I was charged monthly by payroll deduction from when Dr. Cohen did my last exam in February of 2016 through September of 2016 and didn't even get the benefit of my second exam. I would assume Dr. Alvarez knew for a while that he was going to drop Compbenefits. This is very shady.
